Request for Proposals for Franklin County | August 2023

The 25 Years of Service and Impact RFP’s Letter of Intent submission process has closed as of September 12. Applicant organizations will be notified if they are invited to submit a full proposal on September 29.

To commemorate 25 years of grantmaking, the Osteopathic Heritage Foundations are launching a 25 Years of Service and Impact initiative, beginning with a Request for Proposals (RFP) to support organizations in Franklin County addressing critical health and human service needs in the community.

Funding awards of up to $250,000 will be made over a two-year period to selected nonprofit, community-based health and social services organizations and instrumentalities of state serving vulnerable populations in Franklin County. Vulnerable populations include individuals and communities at higher risk for poor health outcomes due to social, economic, medical and/or environmental factors.

Through the RFP, the Foundations will support projects and initiatives aligned with two of its current funding priorities, healthy food access and behavioral health and substance use disorders, as well as additional strategies to improve health outcomes by addressing other social determinants of health.

Successful proposals will document the organization’s history of success in addressing and advancing the goals associated with the funding priorities for the RFP, as noted above. Proposals should demonstrate the ability to achieve the following outcomes:

  • Measurable and sustainable increases in access to health-promoting services for vulnerable populations
  • Contributions to improved health outcomes, over time

In addition, applications should explain how the proposed efforts will advance one or more of the following outcomes during the funding period:

  • Increased and sustained capacity of the funded organization to continue delivering on its mission beyond the funding period
  • Increased financial stability for the funded organization beyond the funding period
  • Increased self-efficacy among clients/populations served
